Engaging Staff by Thinking Outside the Patient Safety Box

Staff engagement is at the heart of patient safety development. To generate excitement for patient safety initiatives, Children's Hospital of Los Angeles (CHLA) has incorporated a maximum fun, maximum learning philosophy into activities targeting their pursuit for zero harm and the journey to become a High Reliability Organization. This presentation will highlight CHLA's activities which include Countdown to Zero Harm Ping Pong Tournament, Game of tHROnes Trivia Game, Safe and Sound Training Program, and much more. Learn about CHLA's RL journey using Risk as their event reporting system since 2011 to today. Through Risk, they have been able to track the impact of their engaging activities and have seen an increase in event reporting and near miss reporting, while also showing a decrease in high severity events.

Utilizing RL6 to develop a Workplace Violence program

Vanderbilt University Medical Center has been working to develop an effective Workplace Violence program. A central element to this program's development has been RL6. From the creation of a new form in RL6, to how the incident data is utilized throughout the enterprise, find out what VUMC has done to build a Workplace Violence program.

Keeping Patients Safe in an Ever-Changing Health Care Environment

Medical malpractice data shows that the number of claims originating from the ambulatory setting are growing at a faster rate than claims involving care provided in the inpatient setting. Pair this with the complexity of providing care to patients across multiple locations, the identification and investigation of safety opportunities can be challenging. Atrius Health has developed a robust patient safety & risk management program that evaluates potential risks and safety concerns within this ever changing environment. Systems were developed to capture safety events occurring in each of these settings as well as a comprehensive review process to develop improvement solutions to help mitigate future risk. It is felt that the development of these systems will significantly contribute to improving overall patient safety, and over time will lead to a reduction in malpractice claims.

Developing Privacy Forms: Because We Care, We're System Aware

Our organization has been on a journey to bring RL and risk reporting to the forefront. On our journey we have identified areas that could benefit from electronic reporting. As an organization we take privacy very seriously and we have identified an opportunity to work with our Privacy Team and make it easier than ever for frontline staff to report a potential privacy breach. We have worked with RL Solutions to develop a privacy form to report Privacy events. The form allows for anonymous reporting and gives our Privacy Team a section to document their investigation, legal and regulatory requirements, and mitigating actions. This will also allow the Privacy Team to streamline their workflow as well as track and trend events for possible improvements.
